Membership Termination Form

You may opt out of your membership at the end of your initial six-week contract. To do so, please submit this Online Termination Form at least three (3) working days before your contract expires and attend an exit interview with our customer care team.

If you do not submit your termination form within this timeframe, your membership will automatically switch to a two-week rolling membership once your six-week term ends. You can still cancel at any time under this rolling membership, provided you give two (2) weeks’ written notice by filling out this same form.
